Certificates are only valid for USA Volleyball sanctioned clubs. Certificates can be used for tryouts, practices, clinics, and tournaments when all participants are current USA Volleyball members.
Certificate of Insurance are a USA Volleyball member benefit.
WHO COMPLETES THIS FORM? Club director or designated club administrator
Membership – must have current Great Plains – USA Volleyball membership
SafeSport training – must be current, cannot expire prior to or during date of event
Background Screen – must be current, cannot expire prior to or during date of event
Complete this form for a Certificate of Insurance for facilities who need an additional insured certificate. Clubs will automatically receive a certificate of insurance when they register their club. No need to request a certificate of Insurance if you only need a certificate for the club.
